Free School Refusal Clinical Mapping Sheet

This free school refusal clinical mapping sheet helps clinicians examine a repeated morning sequence without reducing the problem to attendance alone. It organizes the trigger, distress, child response, caregiver response, school outcome, short-term effect and possible longer-term pattern so that hypotheses can be tested before a return or support plan is chosen.

WHAT IT SUPPORTS

Use the worksheet for a specific clinical task.

  • Mapping a repeated morning avoidance sequence
  • Separating immediate distress reduction from longer-term attendance patterns
  • Clarifying caregiver and school responses without assigning blame
  • Identifying assessment questions before changing the support plan
HOW TO USE IT

Keep the page inside the clinical process.

  1. Choose one recent school morning and describe the sequence as concretely as possible.
  2. Record the child, caregiver and school responses without assuming that any single response explains the whole pattern.
  3. Note the short-term effect and any pattern that appears across repeated mornings.
  4. Use the map to decide what additional assessment, coordination or graded support should be considered.
DISCUSSION PROMPTS

Questions that can keep the page collaborative.

What happens immediately before distress rises?
What changes after staying home, arriving late or entering school?
Which part of the sequence may require information from school or another professional?
PROFESSIONAL BOUNDARIES

What this free template does not do.

  • School refusal can have multiple drivers; this page does not establish a diagnosis or cause.
  • Do not use the map to pressure attendance when safety, bullying, medical needs or another serious concern has not been assessed.
  • Follow local safeguarding, consent and school-coordination requirements when sharing information.
QUESTIONS

Frequently asked questions

Is school refusal always caused by anxiety?

No. Anxiety may be relevant, but attendance difficulties can have multiple contributors. The map is meant to organize the sequence and questions, not determine the cause automatically.

Does this replace a school return plan?

No. It can inform assessment and planning, but an individualized return or attendance plan requires the relevant people and context.

Which paper sizes are included?

The free sample is available in US Letter and A4 versions.
